New Patient Registration

New patients must reside within our catchment area for us to register them. When you contact us you will be requested to complete a new patient questionnaire, a GMS1 form and also provide photo ID (Driving License, Passport etc) together with a utility bill as proof of address. These documents must be returned to the practice in person, with the exception of children when the parent/guardian will be able to register on their behalf.

We work from a well established purpose built surgery in Kesgrave, our building consists of five consulting rooms, three treatment rooms and other reception/administration offices. Our premises are wheelchair accessible and we have a disabled toilet.  

We have a “Hearing Aid Loop” installed in reception to assist patients with hearing problems. Please inform the receptionist if you require this facility.


Change of Patient Details

Please let us know if you change your name, address or telephone number so that we can keep your records up to date. If you leave the area you will have to register with another surgery.
